New Patients

What to Expect?

If this is the first visit with me, your fist visit to a  chiropractor, if you are puzzled /apprehensive and are not sure what to expect, feel free to schedule a 10 minute consultation. My intent is to make sure this is the right place for you to reach your health goals.  And trust me, if it is not, I will let you know.

Individuals who are accepted as patients will have an extensive exam and the result and a recommendation / care plan will be discussed in the following visit.

Visit #1

A thorough Chiropractic Examination including Spinal, Orthopedic, and Neurologic test using a computerized program to determine how your body is functioning. These tests enable us to determine the root cause of your problem and help explain how chiropractic care can help. It also gives us a picture of your overall health.

The latest in computer technology is utilized to locate problems with your spine and their adverse effects on your nervous system and its functions.

These diagnostic tests include a Surface Electromyelogram (sEMG), Digital Thermograph, and Heart Rate Variability (HRV). All are state-of-the-art and completely painless and non-invasive. An email will be sent to you in less than 24 hours with your individualized “core score”. These tests specifically measure Nerve System interference caused by Spinal Subluxations (interference).  Helping us understand how your body functions in addition to how it feels as well as provide objective follow-up assessment of your care to make help monitor your progress.  This initial visit usually takes 45 minutes to and hour so please leave adequate time.

If X-rays are needed, you will be referred out to an imaging center.

Visit #2

Your report of finding is at your earliest convenience, and preferably the day after your examination. During this time I will  go over the findings and explain what they indicate and share with you my treatment / care plan for you.

The reason for doing all this is to carefully assess and treat your case, the cause of your problem rather than only addressing the symptoms. And for those who want to come in without having been a patient of mine asking for a quick “crack” with out an exam or an introduction, you are in luck, there is an abundance of chiropractors in town and I am sure you can find one that would do just that, however that is not how I practice.

New Patient Form

Adults please print and fill out “Adult Sign In” and “Adult Intake”. For individuals under 18, please print Minor Sign In” and “Minor Intake” and bring them with you to your appointment.

Auto Accident Client Forms

What you need to bring with you;

  • Please make sure the accident has been reported to your insurance company and you have an accident claim number
  • Please fill out the bellow forms ( Automobile Injury Intake” and “Assignment of Benefits) prior to your visit and bring them with you to your appointment.
  • Your insurance company’s information; name of the company, policy number and a name of a contact.

Reimbursement?

As of January 2014, I am an out of network practitioner with majority of insurance companies. Pleas call them and learn about your coverage, and please note that having insurance does not grantee coverage, specially when it comes to various services offered through Chiropractic.  I am happy to contact your insurance provider to help clarify things in understanding your coverage, and work with you and your provider to get you the treatment you need. In order to do that I would need at least a 24 hour notice with your full name, DOB, insurance id number and group number. If you are insured through your spouse or parent, I would need their name, their DOB and all the information on their insurance id card as well.

Please be courteous of our time as we are with yours.  A 24 hour cancellation notice is appreciated and in the case of repeated  no-show incidents, we reserve the right to charge a fee of $75 for the missed appointment.  Thank you!
